The Health Workforce Education Center (Pusdiknakes) of the Indonesian Ministry of Health opens and implements Civil Service Education in the Health Sector both at the Secondary Education (JPM) level such as the Health Nursing School (SPK), Midwifery School, Health Analyst High School (SMAK), Dental Care Management School (SPRG), etc., as well as at the Higher Education (JPT) level such as the Nursing Academy (Akper), Environmental Health Academy (AKL) and Electromedical Engineering Academy (ATEM).
Around 1989, the Health Department Schools that were part of the Secondary Education Level (JPM) were converted into Higher Education Level (JPT) called Academies, such as the Nursing School and Midwifery School were converted into the Nursing Academy and Midwifery Academy, the Health Analyst High School became the Health Analyst Academy, and the Dental Care Management School became the Dental Health Academy.
Secondary Education Level Schools (JPM) that were converted into Higher Education Level (JPT) were likely to experience many difficulties in institutionalizing them, so one of the best anticipations at that time to be effective and efficient was to develop their institutions into Surabaya Health Polytechnics (Poltekkes), including 13 Health Academies in East Java which were institutionalized into Surabaya Health Polytechnics in accordance with the Decree of the Minister of Health and Social Affairs Number 1207/MENKES-KESOS/SK/2001 dated November 12, 2001.
The conversion is:
1. The Nursing Academy became the Nursing Department.
* Sutomo Nursing Academy becomes the Sutomo Nursing Study Program.
* Sutopo Nursing Academy becomes the Sutopo Nursing Study Program.
* Sidarjo Nursing Academy becomes the Sidoarjo Nursing Study Program.
* The Tuban Nursing Academy becomes the Tuban Nursing Study Program.
2. The Midwifery Academy becomes the Department of Midwifery.
* Sutomo Midwifery Academy becomes the Sutomo Midwifery Study Program.
* Magetan Midwifery Academy becomes the Magetan Midwifery Study Program.
* The Bangkalan Midwifery Academy becomes the Bangkalan Midwifery Study Program.
3. The Environmental Health Academy becomes the Department of Environmental Health.
* Surabaya Environmental Health Academy becomes the Surabaya Environmental Health Study Program.
* Madiun Environmental Health Academy becomes the Madiun Environmental Health Study Program.
4. The Electromedical Engineering Academy becomes the Department of Electromedical Engineering
5. The Dental Health Academy becomes the Department of Dental Health.
6. The Health Analyst Academy becomes the Health Analyst Department.
And in 2007 the Surabaya Anesthesia Nursing Study Program was abolished, so that the Surabaya Health Polytechnic consisted of 6 departments with 12 study programs.
In 2010 there was a change in leadership within the Surabaya Ministry of Health Health Polytechnic, so it became:
